Before Your Visit
After you’re made your Travel Medicine appointment, you’ll need to do a couple of things to prepare for your visit.
- Print, fill out and bring this Houston Travel Medicine Questionnaire with you to your appointment. If you're unable to bring your Houston Travel Medicine Questionnaire with you, please arrive to the clinic 30-minutes prior to your appointment and a staff member will provide you with a copy upon your arrival. Every traveler in your party should complete a questionnaire.
- Bring your current immunization record.
- If children are traveling with you and they’re not Kelsey-Seybold patients, bring their current immunization records to the appointment.
- Arrive 15 minutes prior to your appointment.
- Bring your trip itinerary.

Travel Kits
Before you leave for your trip, stop by a Kelsey Pharmacy to pick up a Travel Medicine kit curated especially for your destination. Packaged in a convenient tote bag, your kit may include:

Immunizations
Immunizations protect against serious, life‑threatening diseases by preparing the immune system to fight infections. They are especially important for travelers, as certain health conditions can increase risk. If you’re pregnant or have a chronic medical condition such as allergies, asthma, diabetes, or epilepsy, be sure to consult a healthcare professional or a Kelsey-Seybold Travel Medicine professional prior to departure.
There are three types of immunizations:
- Routine
- Required
- Recommended
Routine Immunizations
All adults should update tetanus and diphtheria immunizations every 10 years, even if there aren’t plans to travel.
Required Immunizations
Some countries will require proof of these immunizations before entry:
- Yellow fever *Non-Kelsey-Seybold patients are required to have a good health letter from their PCP to receive the vaccine.
- Meningococcal
Recommended Immunizations
Depending on your itinerary, planned activities and current health status, your healthcare professional may recommend one or more of these immunizations:
- Poliomyelitis
- Influenza
- Hepatitis A
- Hepatitis B
- Typhoid
- Japanese encephalitis
- Meningitis
- Rabies
- Measles, mumps, rubella
- Yellow fever
- Cholera
If traveling with children, additional vaccines may be required for them. Consult with their pediatrician or a Kelsey-Seybold Travel Medicine specialist.
Fee List
Travel Medicine services, including immunizations and consultations, are typically not covered by insurance plans. All services are self-pay, and payment is required at the time of your visit. Appointments are conducted by a Travel Medicine nurse.
Consultation fees are based on appointment length:
- Less than 1 hour: $75
- 1–2 hours: $95
- More than 2 hours: $130
Additional administrative fees may apply depending on the immunization provided.

Travel Checklist
The following checklist will assist you in preparing for international travel. Print this list and check off every item that pertains to your specific travel plans.
- Learn about your destination – type of accommodations, food and water preparation, type of geographical areas you'll visit, and medical services available.
- Visit your local Kelsey-Seybold Travel Clinic or your healthcare provider.
- Get appropriate immunizations for your destination. Obtain an International Certificate of Vaccination or yellow fever card and keep it with your passport.
- Have a dental check-up and complete medical examination, especially if traveling for three months or more.
- Arrange for an adequate supply of your prescription medication.
- Obtain a medical alert bracelet for medical conditions such as diabetes or drug allergies.
- Purchase an extra pair of eyeglasses or contact lenses and sunglasses.
- Obtain a directory of English-speaking physicians.
- Collect items for a first aid travel kit, especially if traveling long-term or off the usual tourist routes, or purchase a travel kit from your Kelsey Pharmacy.
- Check your health insurance coverage.
- Buy additional health/accident travel insurance. Ensure you're covered for any current health conditions (such as heart, pregnancy, etc.).
- Remember your identification and necessary papers (such as passport, visa, etc.).
