To schedule a booster or third dose, simply schedule a COVID-19 Vaccine and let providers know it is for a booster or an additional primary dose.

Who Should Receive a Booster or Third Dose?

Everyone ages 5 years and older should get one booster after completing their initial COVID-19 vaccine series. A second booster is recommended for adults 50 years and older and some people ages 12 years and older who are moderately or severely immunocompromised.

Parent/Guardian Consent Form
If your child is under 18 years old and will be receiving the vaccine without a parent or guardian present, please fill out and sign the Consent to Administer COVID-19 Vaccine Without Parent or Guardian Present (PDF) form prior to your child’s appointment. They will need to bring the signed and completed form to their appointment to receive the vaccine.
