Immediate appointments are available at several locations for COVID-19 Vaccines, Boosters, and Third Doses. Vaccines are free and open to new and current patients who are 5 years and older. Learn more about who should receive boosters and third doses.

Who Should Receive a Booster or Third Dose?

COVID-19 mRNA vaccine boosters are recommended for all patients who are:

  • 12 and older at least five months after a Pfizer vaccine series
  • 18 and older at least five months after the Moderna vaccine series
  • 18 and older at least two months after a J&J/Janssen Vaccine

Patients aged 5 and older who are moderately or severely immunocompromised should get an additional primary shot of Pfizer-BioNTech COVID-19 vaccine (3rd dose) 28 days after their second shot in the vaccine series.

Patients aged 18 and older who are moderately or severely immunocompromised should get an additional primary shot of Moderna COVID-19 vaccine (3rd dose) 28 days after their second shot in the vaccine series.

Parent/Guardian Consent Form
If your child is 12 or 17 years old and will be receiving the vaccine without a parent or guardian present, please fill out and sign the Consent to Administer COVID-19 Vaccine Without Parent or Guardian Present (PDF) form prior to your child’s appointment. They will need to bring the signed and completed form to their appointment to receive the vaccine.
