Ejaculatory Duct Obstruction

Ejaculatory duct obstruction (EDO) is the blockage of one or both ejaculatory ducts, which are the tubules that sperm travels through. An obstruction of the ejaculatory duct can lead to low sperm production, low sperm count, infertility, and post-ejaculatory pain. The condition is the cause of infertility in 1–5% of infertile men.

Symptoms of EDO

Symptoms of ejaculatory duct obstruction may include:

• Painful intercourse
• Pain after ejaculation
• Reduced semen volume
• Blood in semen (hematospermia)
• Pain between the scrotum and anus

Main Causes

The cause of ejaculatory duct obstruction can be congenital or acquired. An example of congenital causes are cystic fibrosis gene mutations. Acquired causes may include epididymitis, past frequent urinary tract infections, scar tissue, and cysts.

Treatment

The most common treatment for ejaculatory duct obstruction is transurethral resection of the ejaculatory ducts (TURED). This procedure, which takes about an hour in an outpatient setting under anesthesia, involves a telescopic device placed through the urethra. Then any obstructions, such as cysts, are removed, if possible.
