Any payment arrangements/plans must be made prior to your procedure date and will require management approval. Please be aware that if your physician determines during your procedure that you need a different procedure(s), or that additional services need to be performed, this could change your financial liability. If financial obligations are not met prior to scheduled procedure, your scheduled time can be altered or the procedure may be canceled.
