FAQ for Marketplace 2025 Patients

Kelsey does NOT accept any plans from: Ambetter, BlueCross BlueShield, Community Health Choice (CHC), Molina, Oscar, and United Healthcare standard plans where Kelsey is not listed in the plan name. There may be additional plans not listed.

No, if you are certain you are enrolled in one of the Aetna or UHC Kelsey-Seybold plans listed above, you do not need to do anything.

1. If you enrolled in a plan that is not accepted at Kelsey and you wish remain our patient, you would need to change plans during Open Enrollment.
2. You will need to refer to the Provider Directory with your new plan and select new providers outside of Kelsey.

If you enroll in a plan that is not accepted at Kelsey-Seybold, your insurance information will be updated in our system and any appointments you have scheduled will be subject to cancellation.

If for some reason you are seen at Kelsey but your insurance is not accepted, you will be financially responsible for all charges.

You may be eligible to continue care with your current provider if you are being treated for a condition that qualifies you to do so. In order to find out if you qualify to continue your care, you must contact your new health plan and request Continuity of Care.

2. You will need to cancel your scheduled appointment.

1. If you are already working with a broker, call that agent as soon as possible.
2. If you want to enroll directly with Aetna – Call 1-844-365-7373.
3. If you want to enroll directly with United Healthcare – Call 1-877-369-2073.
4. If you want to be connected with a broker/agent – Call 713-442-7033 and your call will be routed to a licensed agent who can assist you. Tell them you want to be enrolled in a plan that Kelsey accepts.

You can also contact our Medical Records Department at 713-442-5700 and complete an Authorization for Release of Healthcare Information. Download form here. When you complete and sign the form and return it to Kelsey-Seybold, you authorize the release of your medical records to the community care provider of your choice. You may return the form in two ways:

1. By faxing it to 713-442-5732.
