Rafael Fernandez Bohorquez, MD, graduated with honors from the Universidad del Zulia in Maracaibo, Venezuela, earning his medical degree in 2004. He soon relocated to Illinois, completing his internship and residency at the Chicago Medical School at Rosalind Franklin University in 2009. Dr. Fernandez Bohorquez joined Kelsey-Seybold as a board-certified internist in July 2025 with more than a decade of patient care experience.
Inspired to practice medicine by his parents, who are both physicians, Dr. Fernandez Bohorquez provides primary care to adult patients, serving as the first point of contact for virtually any health concern of his patients and guiding them to the best possible solution. In addition to providing preventive care, he diagnoses and treats most chronic diseases like diabetes, hypertension, mental health concerns, chronic pain, and others, encouraging lifestyle changes in diet, exercise, and stress management with the use of medications when appropriate. Dr. Fernandez Bohorquez is also an addiction medicine specialist who can assist patients with any concern over substance use and any type of addiction. His favorite aspect of practicing medicine is how medicine turns the challenge of solving puzzles into an opportunity to improve his patients’ well-being.

Dr. Fernandez Bohorquez works to understand his patients’ circumstances and priorities as deeply as possible, taking the time to listen and ensure they understand their diagnoses. Then, with a patient-centered and holistic approach, he creates care plans that bring together his patients’ physical, mental, emotional, and spiritual well-being.
Hobbies & Interests
Dr. Fernandez Bohorquez is very interested in music, reading, and exercise. He plays multiple instruments, likes to listen to live music, and enjoys multiple genres, especially rock, pop, and Latin, among others. Dr. Fernandez Bohorquez also reads about science and history and keeps fit by doing functional exercises.
