About Care Teams at Kelsey-Seybold Clinic

Care Teams are a connected team of medical professionals who coordinate a patient’s overall care. Kelsey-Seybold Care Teams can include primary care doctors, physician assistants, nurse practitioners, and specialists.

Advanced Practice Clinicians (APCs), including nurse practitioners and physician assistants, play an important part of our patients’ overall care teams. When you see an APC, you’ll be seeing someone who knows your history and your physician’s philosophy of care. You can schedule appointments with them for preventive care, including physical exams, as well as sick and more acute care needs. They also assist physicians in helping manage their patients’ chronic illnesses, performing in-office procedures, and prescribing new medications or refills.

All Kelsey-Seybold primary care doctors, specialists, and APCs work as a collaborative team who have access to your electronic medical record and can seamlessly coordinate your care and communicate on your behalf. And, if follow-up visits or further consultation with one of our primary care physicians or specialists is needed, our APCs can make referrals and escalate any immediate medical concerns, so your care isn’t delayed.

Advanced Practice Clinicians (APCs) are highly qualified medical professionals who have received advanced medical training and can provide care for everything from preventive care and annual health exams to sick care and management of chronic diseases like diabetes, hypertension, thyroid conditions, and more.

APCs work directly with dedicated Kelsey-Seybold doctors as part of a patient’s care team and are an excellent option when you need care right away. Kelsey-Seybold’s APCs can coordinate directly with primary care physicians and specialists to deliver a timely, coordinated care experience to patients.
