Dr. Tahirih Baker is certified by the American Board of Family Medicine and cares for her patient at Kelsey-Seybold’s West Clinic location.
 
Professional Background
In 1986, Dr. Baker graduated from medical school at the University of Iowa. She completed her residency in 1989 at Baylor College of Medicine where she studied Family Medicine.
 
Academic and Clinical Position
Dr. Baker is co-managing physician for Kelsey-Seybold’s West Clinic. She also has an academic appointment as clinical Preceptor of Baylor College of Medicine’s Department of Family and Community Medicine. Dr. Baker has also served as chairperson of the Medical Standards Committee.
 
Awards and Recognitions
Dr. Baker has been named a “Super Doctor” by Texas Monthly magazine multiple times and has been recognized with a prestigious national certification for excellence in diabetes care, awarded by the National Committee for Quality Assurance (NCQA).
 
Certification through NCQA’s national Diabetes Recognition Program comes after a comprehensive, individualized review process to verify a physician’s adherence to national standards of excellence in caring for patients diagnosed with diabetes. Read more.
 
In addition, Dr. Baker has been recognized for providing outstanding cardiovascular and stroke care by the National Committee for Quality Assurance (NCQA) and American Heart Association/American Stroke Association (AHA/ASA) Heart/Stroke Recognition Program. Recognition through the Heart/Stroke Recognition program is achieved as the result of a comprehensive, individualized review process that verifies a physician’s adherence to national standards of excellence in caring for patients with cardiovascular disease or who have had a stroke.
