Immunization Checklist

Here's a quick summary of new immunizations needed for entry to school this fall:

  • Two doses of the varicella vaccine, two doses of the MMR vaccine and two doses of the hepatitis A vaccine are required for kindergarten entry.
  • Two doses of the varicella vaccine and a booster dose of the Tdap vaccine are required for 7th grade entry.
  • The meningococcal (Menactra) vaccine is required for any child who will be 11 or 12 by the start of the school year.

In addition to these new requirements, refer to the chart below for a list of required immunizations needed before the start of the school year. For more information on the 2009 immunization schedule, visit CDC.gov.
